Social Outreach

OBJECTIVE   ​

Chelsea EDC is a partnership with Rise Up AC, a non-profit organization dedicated to assisting at-risk individuals experiencing homelessness. Recognizing the importance of addressing the needs of vulnerable community members, Chelsea EDC and Rise Up AC will work together to provide comprehensive social services, focusing on individuals who may have difficulty accessing existing support networks.

IMPLEMENTATION:

• Engage and provide support to individuals experiencing homelessness or at risk of homelessness who may not receive adequate assistance through existing social service networks.

• Comprehensive case management services address the complex needs of individuals, including substance use treatment, mental health services, transportation, emergency shelter, food, and counseling.
